For each situation below, choose how likely you would be to doze off or fall asleep — not just feel tired. Answer for your usual way of life in recent times, even if you haven't done some of these things lately.

0 = would never doze 1 = slight chance 2 = moderate chance 3 = high chance
1. Sitting and reading

4. As a passenger in a car for an hour without a break
5. Lying down to rest in the afternoon when circumstances permit
